FYI: Agreement Benefits FAQ

I need a letter that lists my insurance coverage/indicates I have lost coverage.

Can you provide my medical ID cards?

  • Health Insurance – Please contact your plan administrator. (See below for contact information)
    Dental Insurance: Go to www.ytth.com, click “Explore your benefits”, click Dental – takes employee to site that can answer all dental related questions and there is a link to the 24/7 member website where employees can access their ID card. Employees can also contact AETNA at 1-877-277-3368.
    Vision Insurance: Go to www.ytth.com, click “Explore your benefits”, click Vision – takes employee to site that can answer all vision related questions and there is information about the EyeMed App where employees can access their ID.

How do I pay for my health coverage while on medical leave?

  • Employees will receive a letter from their insurance provider explaining continuation of coverage. The letter explains COBRA options, as well as when coverage will end if POD is not provided. Generally, the H&W cost sharing will cease when an employee is off due to medical leave for self.

Why has my insurance coverage ended?

  • Something could be amiss regarding the National Health and Welfare Agreement. Please create a ticket to be researched further using the link found below.

How can I change my life insurance beneficiary?

  • Beneficiaries must be updated through MetLife. There is a form to print and fill out that can be found on the National Plan website – www.ytth.com

    If employees are also enrolled in voluntary life, please call MetLife at 1-866-659-1377 to update both plans.

Why was I was charged for health insurance twice this month?

  • Common scenarios:

    Employee has switched from agreement to non-agreement and met requirements to be eligible for coverage under both plans for the respective month.

    Employee has returned from a leave and owes for a previous Health and Welfare cost sharing deduction.

    If that is not the case, please create a ticket.

Questions referencing voluntary benefits

  • Voluntary benefits are administered through BenefitHub. For questions, please contact them at 1-866-664-4621 or email customercare@benefithub.com. To access the website, employees activate their account using their employee ID and zip code.

I am switching from non-agreement to agreement and need to know my benefit options.
